Which Fields do I Need to Fill Out for my Agency's Information? Print

In order for us to export the user’s report to the state we need the following information:

1. Agency Name

2. Agency Certification Number

3. Type of Service

4. Agency Type

5. Agency Number

6. Agency State.

Note: If the user is an education or events agency the user just needs to fill in the Agency Name.
